Care Manager/Assessor Aging
In anticipation of future openings, Office of Human Services, Inc. Area Agency on Aging for McKean, Elk and Cameron area is seeking applicants for full-time Care Manager/Assessor. Duties include assessing the needs of older individuals, developing care plans to help meet the identified needs and managing ongoing care to help them remain at home.
Openings may be based in the Bradford, Ridgway or Smethport community.
Either: Bachelor’s degree in social services, human services, nursing, or related field
Plus 1 Year of experience in social service or health care related setting.
Or: any equivalent combination of experience and training must have 12 credits in sociology, social welfare, psychology, gerontology or other related social sciences
Excellent communication and documentation skills required. Case records digital, therefore computer skills are essential.
Resume should include detailed information about any experience assessing health or social needs, developing care plans to meet those needs, performing investigations or managing care. Specify pay requirements.
